Job Overview: 


We are seeking a dedicated and experienced Care Home Manager to lead our Care Home Department. This role oversees medication management coordination, team leadership, and operational performance across multiple locations.

Responsibilities: 



Coordinate medication management with providers, caregivers, and case managers
Ensure compliance with company policies and regulatory standards
Lead and train Care Home department staff
Monitor department performance and track KPIs
Prepare reports on patient enrollment, referrals, and service trends
Troubleshoot patient and provider concerns



Education and Experience: 



Degree in Social Work, Nursing, Social Sciences, or related work experience
2+ years experience in public health or related field
1+ year hands-on case management experience
Strong organizational and communication skills
Bilingual preferred - English and Ilocano/Tagalog



Working Conditions: 




Full-time, typically requiring 40+ hours per week 






Work is performed in an office setting 








Prolonged periods sitting at a desk and working on a computer 






Must be able to lift up to 15 pounds at times 





May require travel between locations and visits with care givers

Job Overview:
The Pharmacy Technician supports pharmacists in dispensing medications, managing inventory, and providing exceptional customer service to patients. This role involves accurately preparing and labeling prescriptions, assisting with medication inventory management, and ensuring compliance with all regulatory standards. The Pharmacy Technician plays a critical role in maintaining the efficiency and effectiveness of the pharmacy operations.
Responsibilities:
1. **Prescription Preparation and Dispensing**
   - Assist pharmacists in preparing and dispensing prescription medications
   - Accurately measure, count, and label medications for patient prescriptions
   - Operate pharmacy equipment such as pill counters and label printers
2. **Customer Service**
   - Provide friendly and efficient customer service to patients and healthcare providers
   - Answer patient inquiries regarding prescription status, medication instructions, and pharmacy services
   - Handle prescription pick-up and drop-off, ensuring accurate patient information and payment processing
- Perform patient outreach and communicate with patients effectively to close gaps in their medication therapy.
3. **Inventory Management**
   - Manage pharmacy inventory, including ordering, receiving, and stocking medications and supplies
   - Conduct regular inventory audits to maintain accurate stock levels and prevent shortages
   - Monitor expiration dates and remove expired or recalled medications from inventory
   - Assist with the organization and cleanliness of the pharmacy storage areas
4. **Data Entry and Record-Keeping**
   - Enter prescription information into the pharmacy management system accurately and efficiently
   - Maintain accurate patient profiles and medication records
   - Process insurance claims and resolve billing issues with insurance companies
   - Ensure compliance with all legal and regulatory requirements related to prescription processing and record-keeping
5. **Medication Safety and Quality Assurance**
   - Adhere to safety protocols and best practices in medication handling and dispensing
   - Assist in monitoring for potential medication errors and discrepancies
   - Participate in quality improvement initiatives to enhance pharmacy services
   - Support pharmacists in conducting medication reviews and utilization reviews
6. **Supportive Duties**
   - Assist pharmacists with administrative tasks, including scheduling and workflow management
   - Participate in staff meetings and training sessions to stay updated on pharmacy policies and procedures
   - Provide training and guidance to new pharmacy technicians and interns
   - Perform other duties as assigned by the pharmacist or Pharmacist-in-Charge
Required Skills:
