We are seeking a compassionate and skilled Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our healthcare team in providing high-quality, patient-centered primary care. In this full-time, permanent role, you’ll serve a diverse patient population by performing comprehensive health assessments, managing acute and chronic conditions, and collaborating closely with multidisciplinary care teams to deliver seamless, effective care. Schedule: Monday through Friday, 8:00 a.m. – 5:00 p.m.

Essential Duties and Responsibilities:

• Perform comprehensive patient assessments, including initial evaluations, routine physical exams, and ongoing monitoring.
• Develop, implement, and adjust care plans based on clinical findings and patient goals.
• Diagnose and manage common acute and chronic health conditions within the FNP/PA scope of practice.
• Prescribe and manage medications in accordance with the institutional formulary and physician-delegated authority.
• Provide end-of-life support and symptom management for patients and families as appropriate.
• Coordinate patient care with specialists, hospitals, and community resources to ensure continuity of care.
• Educate patients and families on health promotion, disease prevention, medications, and care plans.
• Document all patient interactions accurately and in compliance with regulatory and organizational standards.
• Maintain clinical competencies through ongoing education and training.

Education and Experience:

• Current Texas licensure as a Family Nurse Practitioner (APRN-NP) or as a Physician Assistant (PA)
• Current certification through the appropriate national board (NCCPA for PAs)
• Minimum of 1 year of experience in a primary care or family practice setting preferred
• Bilingual fluency in English and Spanish strongly preferred
