This is a national leadership role requiring strategic vision, operational excellence, and strong clinical credibility. The CMO will help Medrina evolve from a collection of high-performing provider groups into a cohesive, integrated medical enterprise that defines the standard for post-acute medicine in the U.S. This role will serve as the clinical architect and operator of Medrina’s post-acute medicine platform. The CMO will design and implement standardized care pathways, drive measurable improvements in outcomes (e.g., ALOS, readmission reduction, and functional recovery), and ensure seamless collaboration between hospitalists, internists, physiatrists, and advanced practitioners. This role will lead Medrina’s transformation into a nationally scalable, integrated care delivery model—bridging Internal Medicine and Physiatry to improve patient outcomes, reduce avoidable hospitalizations, and maximize value-based care performance. The CMO will collaborate with various clinical and operational leaders across multiple service lines and will report to Medrina CEO. The CMO will work remotely from their home office with occasional travel.
Essential Duties and Responsibilities:
Strategic Leadership
- Define and operationalize Medrina’s integrated post-acute clinical model, connecting hospital-based care, SNF medicine, and outpatient transition management.
- Partner with executive leadership and clinical operations to establish national standards of care that align with ACO/VBC performance goals.
- Serve as the clinical face of Medrina’s post-acute strategy in external engagements with ACOs, payors, and health systems.
Clinical Integration & Pathways
- Lead the design and rollout of evidence-based care pathways across SNFs and post-discharge settings.
- Promote collaboration between Internal Medicine and Physiatry, ensuring cohesive patient management that supports functional and medical recovery.
- Implement data-driven performance dashboards to monitor outcomes, readmissions, and utilization metrics.
Value-Based Care & Quality
- Drive success within current and future LTC and high-risk ACO programs, ensuring clinical and financial alignment across all stakeholders.
- Evaluate and refine clinical systems (EMR, telemedicine, analytics) to optimize care delivery and ACO savings potential.
- Oversee quality assurance (QA/QI) programs, ensuring compliance with CMS and regulatory requirements while promoting provider accountability.
Clinical Leadership & Culture
- Lead, mentor, and align Medrina’s Internal Medicine provider base around standardized practices and shared quality goals.
- Build a culture of collaboration, transparency, and continuous improvement across the national network.
- Partner with regional medical directors and operations teams to identify best practices and deploy scalable models in new markets.
Job Requirements:
- MD or DO with 10+ years of clinical experience in post-acute settings (SNF, LTAC, or transitional care) with proven track record of integrating multiple specialties (i.e. IM and Physiatry) under a unified clinical model.
- 8 years of experience as a Medical Director or system level physician executive managing interdisciplinary teams in Skilled Nursing Facilities with deep understanding of SNF dynamics, care transitions, and provider alignment models.
- Demonstrated leadership in ACO, VBC, and population health environments.
- Proven record in QA/QI initiatives and outcomes improvement.
- Proficiency with data analytics, EMR optimization, and telemedicine in post-acute care.
- Active engagement in national post-acute organizations (e.g., AMDA, AMDA QI committees).
- Strong communication and change-management skills—able to influence clinical practice across diverse markets.
