POSITION OBJECTIVE:

The Family Assessment Specialist will oversee the referral process and assess individuals referred to the program. Working alongside other team members, they will assist with locating services and resources in the community. The Family Assessment Specialist will assist with the exploration and identification of personalized goals of each client. This position will also track family and person-level data and participate in quality review processes.

ESSENTIAL FUNCTIONS:

· Screen and follow-up on referrals for Peer Support Services.

· Conduct initial needs assessment, review the individual’s SUD history, and identify the individual’s goals and expectations.

· Together with Peer Support Specialists, develop individualized, comprehensive treatment and recovery service plans.

· Maintain accurate and up-to-date records and standardized data on all individuals

· Systematically review caseloads with the supervisor each week, focusing on new individuals and those who are having difficulties.

· Negotiate and create opportunities for access to treatment and social services for individuals.

· Assist with data collection for quality assurance and program evaluation.

· Assist with locating new community-based services, programming, activities, and support.

· Ability to safely & successfully perform essential job duties consistent with the ADA, FMLA, and other federal, state, and local standards.

· Ability to maintain reasonably regular, punctual attendance consistent with ADA, FMLA, and other federal, state, and local standards.

· Ability to comply with all personnel policies and procedures.

· Other duties as assigned.

 

MINIMUM REQUIREMENTS:

High school diploma or equivalent

 Experience receiving treatment and services for substance use disorder.

· Lived and work experience in the child welfare system

· Candidate must demonstrate an understanding of, and belief in, the addiction recovery process

· Registered with the Florida Certification Board as a Certified Recovery Peer Specialist OR meets the qualifications to become certified.

· Cultural and linguistic competence and sensitivity to population served.

· Knowledgeable about community resources in the central Florida area.

 

SPECIAL SKILLS & OTHER REQUIREMENTS:

· Ability to work independently as well as part of a multidisciplinary team of social workers, treatment providers, community support workers, and the judiciary.

· Flexibility to adapt to unforeseen needs or circumstances.

· Excels at problem solving and multi-tasking, organized, efficient and goal directed.

· Ability to maintain effective working relationships with patients/families and staff.

· Excellent interpersonal, written and verbal communication skills.

· Effective use of the Internet and Microsoft Office programs such as Word, Excel, PowerPoint and Outlook.

· Ability to handle confidential information.

 

PREFERRED:

·Current resident of Orange, Osceola, or Seminole County preferred

 Certified Recovery Peer Specialist

· Degree in Social Work or related human services field

· Bilingual (Spanish and/or Haitian/Creole)

 

SPECIAL CONDITIONS:

· Thorough Background Screening upon hire including FBI, FDLE, Local Law check, Employment Reference check, DMV check, Criminal Records check, Social Security Trace & Drug Screening. FDLE check will be run at least every 5 years.

· On-call assignments- Yes

· Travel- local: 50% per week, long-distance: 1% per month

· Attend at least 15 hours of in-service training annually
