The After-Care Case Manager II, will work with a team of staff to help create a safe, healing environment for guests in a Christian transitional living program. You are part of a team that functions with internal and external partners for transitional and permanent housing. The team will also consist of case management, mental health, and spiritual counseling.
Scheduled: Monday – Friday, 8:00AM - 4:30PM
Target starting pay: $27.42/hour
How you will serve
• Assists with training new staff and providing training to other team members.
• Collaborate with UGM program ministries to ensure After-care is implemented in a timely manner for guests.
• Develop housing stability plans in partnership with guests and outside housing resources as needed.
• Help facilitate and organize community events for guests and alumni.
• Work with a Relationship Manager to create partnerships with other social services to assist our guests in transitional living as needed.
• Follow-up and develop on referrals made on or with Relationship Manager.
• Manage relationships with like-minded organizations to provide services to our guests.
• Support guests with finding work, budgeting, and guests’ responsibilities.
• Seeking to develop a continuing knowledge of the field, what services are provided in the community, and
• establishing communication with those entities
• Create data and narrative program reports on guest outcomes and programmatic successes.
• Ability to address and quickly solve problems with staff, volunteers, as well as guests, and be able to intercede and find reasonable solutions in order to maintain harmonious relations.
• Facilitate Community visits with clients as assigned, to include signing up clients to meet with community events and providers.
• Process the intake assessments and screening of guests.
• Assists with finding, securing, and maintaining permanent housing, and helps clients to reintegrate into the community.
• Drive company vehicles for client transportation or other work-related activities.
• Manage a caseload of individual guests in a transitional after-care program.
• Meets with guests as needed and maintains all electronic and paper client files.
• Provide crisis intervention information, chemical dependency case management, and referral help.
• Communicate with all staff through the Client Tracker (CT11).
• Help coordinate and facilitate support groups, Bible studies, addiction recovery curriculum, and classes.
• Develop and maintain ongoing guest files.
• The administrator observed urine collections for guests.
• Effectively articulate a case for support so that others take heart to our vision/mission/goals.
• Perform welfare checks at guest residences.
• Help maintain a positive, pleasant, and Christian atmosphere at the facility.
• Participate in organizational meetings, training, and initiatives as required.
• Perform other duties assigned to support the Mission’s success.
How you qualify
• An Associate Degree is required. A combination of education and four years of relatable experience may be substituted.
• Three years of experience working in a social services or rescue mission setting. One of those years must be in a Case Manager role.
• Proficient in Microsoft Office Suite
• Possess a valid Washington State Driver’s License and be able to meet the standards of the Mission driver criteria is preferred, including but not limited to:
o Clean driving record.
o No current suspensions or revocations.
o At least 21 years of age

Work Environment
While performing the duties of this job, the employee is frequently required to stand and/or walk, balance, twist, climb ladders, stoop, kneel, perform overhead work and reaches with hands/arm. Frequently work for extended periods of time in indoor and outdoor settings with varying temperature environment. Frequently uses judgement and memory to make decisions. Occasionally you will sit and keyboard. Occasionally lifts up to 50 pounds and walks, use stairways and climb to multiple floors carrying tools and equipment. Specific vision abilities required by this job include close vision, color vision, distance vision, depth perception, and ability to adjust focus.
