This is a full-time role operating from central or eastern regions of United States during standard business hours, Monday - Friday (from your home office remotely) with some travel in the United States for company meetings (this is not a flexible schedule opportunity). This role handles sensitive information; work must be performed in a private and secure setting (with a door) requiring reliable internet and phone connectivity.  The candidate must reside in the Central or Eastern time zones/region of United States.  This role does not offer immigration visa sponsorship.

This role offers annual salary of $125,000 with an annual performance bonus of up to 10%.  We offer an excellent benefits package including 15 days of vacation, 7 paid holidays, and 5 sick days annually, multiple medical and vision insurance plans, which begin day one (no waiting period), employer-paid life insurance, 401(k) with a company match, $1,700 annual professional development reimbursement, $100 monthly teleworker stipend and more.

The Director of Telehealth & Remote Patient Monitoring (RPM) Services is responsible for the development, implementation, operational oversight, compliance, and performance of the organization's Telehealth and RPM programs.  This position will lead the integration of RPM monitoring and clinical alerts with the organization's Clinical Contact Center, after-hours escalation processes, provider practices, Care Management programs, and ACO/Population Health infrastructure.  A key responsibility of this role is ensuring that clinical activity occurring overnight is appropriately reviewed the following day and communicated to the responsible providers, practice leadership, Care Management, and ACO teams. The Director will establish a closed-loop process so that significant alerts and escalations result in appropriate follow-up, documentation, care coordination, and resolution.  The Director will also work closely with Clinical Leadership, Compliance, Revenue Cycle, Information Technology, and Operations to ensure Telehealth and RPM services are built and operated compliantly and that services are appropriately documented and billed.

Key Responsibilities:

Telehealth & RPM Program Development:

  • Lead the development, implementation, and expansion of the organization's Telehealth and RPM programs. Establish standardized operating models, policies, workflows, patient enrollment processes, clinical monitoring standards, documentation requirements, and performance expectations.
  • Ensure programs are designed to be scalable across multiple practices, facilities, patient populations, and geographic markets.

Clinical Contact Center Integration:

  • Lead the operational integration of RPM and Telehealth services with the Clinical Contact Center and after-hours coverage model.
  • Partner with Contact Center and Clinical Leadership to establish clear processes for the identification, prioritization, communication, and escalation of clinical alerts to the appropriate APP, physician, or emergency level of care.
  • Ensure Telehealth capabilities are incorporated into the escalation model when remote clinical evaluation is appropriate.

Overnight Review & Next-Day Clinical Handoff:

  • Oversee the review and reconciliation of significant overnight clinical activity, including RPM alerts, Contact Center escalations, Telehealth encounters, changes in condition, emergency transfers, and other events requiring continued follow-up.
  • Ensure clinically significant events are communicated to the appropriate practice provider, operational leadership, Care Management team, and ACO/Population Health team and that responsibility for follow-up is clearly assigned.
  • Establish accountability for closing the loop on outstanding clinical issues rather than allowing the overnight response to become the endpoint of care.

Care Management & ACO Integration:

  • Integrate RPM and Telehealth information into the organization's broader Care Management and population health strategy.
  • Develop processes to identify patients who may benefit from additional Care Management intervention, provider follow-up, medication review, care-plan modification, additional monitoring, or closure of identified gaps in care.
  • Ensure relevant information is communicated to the ACO and Population Health teams for attributed patients when appropriate.

Regulatory & Compliance Oversight:

  • Partner with Compliance, Legal, and Clinical Leadership to ensure Telehealth and RPM programs operate in accordance with applicable federal and state requirements.
  • Maintain working knowledge of requirements related to RPM, Telehealth, patient consent, provider licensure, scope of practice, supervision, documentation, privacy, data security, and other applicable regulatory standards.
  • Support the development of state-specific requirements as programs expand into additional markets.

Billing & Revenue Cycle:

  • Partner closely with Revenue Cycle, Coding, Compliance, and Finance to ensure Telehealth and RPM services are appropriately documented, coded, and billed.
  • Maintain a strong working knowledge of RPM and Telehealth reimbursement requirements and ensure operational workflows support compliant billing.
  • Monitor billing capture, documentation compliance, claims performance, denials, and potential overlap with other Care Management services such as CCM and PCM.
  • Work with Revenue Cycle and Compliance to identify and correct operational issues that could result in missed revenue or inappropriate billing.

Clinical Escalation & Quality:

  • Partner with Clinical Leadership to develop and maintain standardized clinical escalation protocols for RPM and Telehealth services.
  • Ensure that responsibilities are clearly defined across monitoring staff, Contact Center personnel, APPs, physicians, and daytime clinical teams.
  • Monitor clinical escalations, unresolved alerts, emergency transfers, Telehealth utilization, and follow-up performance to identify opportunities to improve patient care and operational effectiveness.

Technology & Systems Integration:

  • Partner with Information Technology, vendors, and operational leadership to integrate RPM, Telehealth, Contact Center, EHR, secure communication, Care Management, and Population Health systems.
  • Identify opportunities to automate communication and reduce manual processes while ensuring critical clinical information is reliably transferred between systems and teams.

Program Performance & Growth:

  • Develop and monitor performance measures for the Telehealth and RPM programs, including patient enrollment, monitoring participation, alert volume, clinical escalations, Telehealth utilization, follow-up completion, billing performance, patient outcomes, and financial performance.
  • Use data to identify opportunities to improve workflows, expand eligible patient populations, strengthen clinical outcomes, and improve program sustainability.

Job Requirements:

  • Bachelor's degree in Nursing, Healthcare Administration, Public Health, Health Information Technology, Business Administration, or related field required; advanced degree preferred.
  • Clinical background, RN, or APP.
  • Minimum of 7 years of progressive healthcare operations, clinical operations, population health, Care Management, Telehealth, RPM, or related experience.
  • Experience developing, implementing, or managing technology-enabled healthcare programs.
  • Strong understanding of physician and APP clinical workflows.
  • Working knowledge of Medicare reimbursement, Telehealth, RPM, and professional billing.
  • Experience working with clinical escalation and Care Management processes.
  • Strong project-management, analytical, and performance-management skills.
  • Ability to work collaboratively across Clinical, Operations, Compliance, Revenue Cycle, IT, and Population Health teams.

Preferred Experience:

Experience with RPM, Telehealth, physician practices, post-acute care, senior living, CCM/PCM, ACOs, Population Health, Medicare Advantage, value-based care, or multi-state healthcare operations is highly desirable.

Physical Requirements:

  1. This is a full-time remote, work-from-home position, primarily working during standard business hours handling sensitive and confidential information. Work must be performed in a private and secure setting requiring reliable internet and phone connectivity.
  2. Ability to communicate via virtual/online meetings daily with a camera on as well as being responsive in a timely manner during work hours via email, MS Teams and phone.
  3. Ability to travel quarterly for training, and meetings.
  4. Ability to sit, stand, and walk for extended periods, lift and move up to 25 pounds.
