Position Summary:  
Supervises field Care Management teams, delegates duties to Care Managers and responds to clinical operational issues/concerns while promoting quality and productivity in the administration of health services.

Essential Functions:
1.         Manages the day-to-day operations of work assignments for direct reports to ensure adherence of standard goals/metrics for quality and performance. 
2.         Assists the Clinical Director with development and on-going refinement of tools and procedures used by APS Healthcare.
3.         Develops and maintains the Care Management Team, who assure that participants are receiving services in an appropriate and timely manner.
4.         Assists with problem-solving on clinical and contract-specific issues.
Provides clinical training of Care Management teams. 

Education:
  ·         Masters prepared in Social Work with licensing (such as LCSW, LSW, LGSW)

Experience: 
·         Minimum 4-6 years experience in a medical setting required, which will include 2 years experience in direct patient care required.
·         Minimum 3-4 years management experience required.
·         Public sector experience preferred required

Knowledge:
·         Thorough knowledge of managed care and medical management processes and strategies
·         Decision making skills
·         Customer service orientation with good interpersonal skills

Skills:
·         Excellent clinical assessment skill; strong written and verbal skills
·         Leadership skills
·         Prioritization and organizational skills
·         Proficiency in care management application systems
·         Proficient at Microsoft word and Excel
