We are seeking an experienced Nurse Practitioner (NP) to provide high-quality, focused care in 1-2 nursing facilities. In this PM&R-centered role, you will round on patients, identifying and treating barriers to recovery such as mobility issues, pain management, wound care, functional decline, and rehabilitation needs. This position emphasizes efficiency, autonomy, and balance, allowing you to deliver targeted interventions without the demands of extended hours or emergencies.Key Responsibilities

Conduct rounds in assigned nursing facilities, seeing patients twice per week to monitor progress and address recovery barriers in a PM&R framework.

Manage an average of 5-15 patient encounters per day, with each interaction lasting approximately 5-7 minutes for focused assessments, treatments, and care planning.

Collaborate with interdisciplinary teams (e.g., physicians, therapists, and facility staff) to develop and implement individualized recovery plans.

Perform charting and documentation remotely from home using our efficient electronic health record (EHR) system.

Maintain compliance with all regulatory standards, including those specific to PM&R and post-acute care.

Decide and maintain your own consistent rounding schedule within daytime weekday hours, providing flexibility while ensuring patient needs are met.

- Required Skills

-Master's degree in Nursing and current certification as a Nurse Practitioner (NP) from an accredited program.
-Active, unrestricted NP license in [relevant state(s)], with prescriptive authority.
-Minimum of 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field; experience in nursing facilities strongly preferred.
-Strong clinical skills in assessing and managing barriers to recovery, including rehabilitation, pain, and chronic conditions.
-Excellent communication and time-management abilities to handle brief, efficient patient encounters.
-Proficiency with EHR systems and remote documentation.
-Ability to work independently with a high level of autonomy and consistency.
